Upon completion of your medical history, our doctor will take the time to thoroughly review your history to ensure you are treated safely.

Your dental history will allow our doctor to learn more about your previous dental experiences and identify opportunities to make your experience more pleasant.
PRIMARY Concerns
Your primary concerns will enable our doctor to address the main reason for your visit and help alleviate any pain or discomfort.
Treatment Plan
Once our doctor has completed your comprehensive examination, a treatment plan will be deduced to guide you through your treatment.
